Nigerians will be required to produce court affidavits and police reports before they can get replacements for missing or damaged COVID-19 vaccination cards.
This was contained in a document published by the National Primary Health Care Development Agency and made available to our correspondent on Sunday.
The Agency usually issues vaccination cards to citizens who get vaccinated. The card serves as proof of vaccination.
Checkout Magazine reported plans by the Federal Government to make vaccination mandatory for civil servants and others who may want to access government offices.
The NPHCDA in its directive, stated that citizens who lose their cards would have no choice but to produce court affidavits and police reports.
The directive read, “The client will need to visit the vaccination site where he or she took the first dose of COVID-19 vaccination with a police report, sworn affidavit detailing the following: first name, middle name, date of birth and telephone phone number used during vaccination.
“Others are vaccination ID (if available), date of first dose (if known), date of second dose (if taken) and photo of lost card (if available).
“Once this information is provided and the client’s record is found either on the physical record or the EMID portal, a new card will be issued without delay at that same vaccination site.”
